A new article published in Pediatric Emergency Care reported that sigmoid volvulus (SV) is an extremely rare cause of abdominal pain in children. The article described that SV occurs more frequently in older adults, when a redundant loop of sigmoid wraps around its elongated, narrow mesentery causing obstruction and ischemia to the affected bowel segment. In children, it usually presents with abdominal pain, nausea, and abdominal distension. While presentations may be acute or chronic with a history of episodic constipation or abdominal distension. This article further discussed that treatment plan for SV includes an initial reduction of the volvulus via sigmoidoscopy with rectal biopsy to rule out Hirschsprung’s disease. Yet, operative management to remove the dilated sigmoid colon may be necessary in the event of recurrence or confirmed Hirschsprung’s disease. In inference, it was stated that although rare, SV should be considered in a child presenting with abdominal pain since a missed diagnosis can impose high risk of morbidity and mortality.
